HeartWell is a free health education program designed for seniors to increase awareness about preventative guidelines, symptom recognition and management, medications, blood pressure monitoring, diet, and lifestyle enhancements. The program is staffed by a Suburban Hospital registered nurse at three senior centers in Montgomery County: Gaithersburg, Holiday Park, and Margaret Schweinhaut.

The HeartWell Program:

A unique, nationally recognized holistic approach assisting seniors to maintain their health and independence.
A free health maintenance program designed to assist individuals to optimize wellness and manage illness.
Conveniently located in three Montgomery County Senior Centers.
Assists participants in maintaining as high a level of functioning as possible.
An educational program designed for seniors to increase awareness about preventative guidelines, symptom recognition and management and lifestyle enhancements.
Staffed by Suburban Hospital Registered Nurses, the HeartWell program provides nursing consultations, blood pressure monitoring, wellness programs and health education.

Visit any of our clinics for free blood pressure screenings and health counseling by a Suburban Hospital Registered Nurse.

Know Your Numbers Program:

Suburban Hospital and Montgomery County Senior Programs sponsors the "Know Your Numbers" program to teach individuals with cardiovascular disease, high blood pressure, diabetes, or other chronic diseases how to manage their symptoms and disease by focusing on education concerning early disease symptom recognition, symptom management, medication, diet, and lifestyle changes.

During certain days and hours, you can meet with our registered nurses for counseling on the numbers that are important to managing your heart health.
