Potomac Home Health Care, a non-profit joint venture of Sibley Memorial Hospital and Suburban Hospital, is a full service, Medicare-certified home health agency providing a comprehensive range of home care services in Maryland and Washington, D.C. Working with a team approach, we make every attempt to recognize and meet the needs of the homebound patient.

Our mission is to meet the needs of the community by providing cost efficient quality home health care and patient education services to assist patients in achieving their maximum independence and/or an enhanced quality of life. Our program stresses personalized care, tailored to the individual patient's needs. We work closely with the physician, the patient and family members in planning, reviewing progress and making adjustments to the plan of care. This approach results in a better treatment plan and greater understanding and support for the patient and family. Our program has the unique capability of offering the resources of two full service hospitals. Our services include:
