Suburban Hospital participates in a wide range of health plans. We accept all private pay patients and the insurance plans listed below. Before scheduling an appointment or seeking treatment, please contact your insurance provider to ensure full coverage.
Our Patient Access staff will verify your insurance coverage and work with your physician's office to obtain authorization from your insurance carrier, when necessary. Please note that your insurance company may not pay for all of your services if your benefit plan requires out of pocket expenses to be paid (such as, deductibles, co-payments, and co-insurance) or if you are required to seek services (such as lab, radiology, etc.) at specific locations/companies. Deductibles and co-payments will be collected at the time of service. For your convenience, we accept VISA, MasterCard, American Express, and Discover credit cards or a personal check made payable to Suburban Hospital.
If you do not see your health insurance plan listed below, or if you have any questions regarding Suburban Hospital's participation in your health plan, please send us an email to insurancequestions@suburbanhospital.org. We will respond to your inquiry as quickly as possible.
Professional Fees (physician or other provider fees) are separate from the hospital fees. You may receive bills from multiple physicians for their services, including but not limited to your private physician, anesthesiologist, pathologist, radiologist, cardiologist, emergency room physician, and other specialists who participated in your care. If you have questions regarding the bill for their services, please contact the individual provider. Please note that Suburban Hospital cannot guarantee that private physicians participate with your health insurance plan.
Suburban Hospital participates with the following health insurance plans, although not every procedure is covered by every plan. Please check with your plan for individual coverage information.
